ATTENTION ALL JUNIORS, PARENTS & GUARDIANS:

If you are a Junior and are therefore travelling with a Parent/Guardian, they also must fill out this form.

The Parent/Guardian you are travelling with must fill out the PARENT/GUARDIAN FORM for each Junior they have in their care.
If you are not travelling with your legal Parent/Guardian (i.e. 16 yo boy travelling with teammate and his parent), then your legal Parent/Guardian must fill out the PARENT/GUARDIAN FORM nominating another adult as the guardian of the Junior.
